This diagram illustrates the phase relationship between the primary and secondary voltages and currents in a transformer. The output can be either in-phase or out-of phase with the input depending on the relative direction of the primary and secondary windings.

Notice how the primary and secondary waveforms rise and fall together when the secondary voltage is in-phase with the primary and how they are opposite when the secondary voltage is out of phase with the primary.
